Please complete the form below with your details. We will then send you your Deed Poll documentation by first class post (same day dispatch for orders received before 4pm weekdays). The total cost, per adult, including postage is £16 (additional stamped legal copies £2 each).

You will receive (sent in a card backed A4 size envelope for protection): your stamped legal UK Deed Poll document, professionally presented and printed on high quality 100gsm cream paper (plus any extra stamped legal copies ordered), clear instructions detailing how to sign your Deed Poll, covering letter template, list of common places to notify, and secure digital archiving.

Plus, our free downloadable guide to informing Government Departments and other organisations that you have changed your name (includes contact/address details for most major organisations).

N.B. You can change any part or all of your name, including fore, middle and surnames. Data Protection Act 1998 Registration No. Z2723405. We accept all major UK credit and debit cards.
