How do I change the name on my Passport?
Most people choosing to change their names or that of a child's by Deed Poll will have a UK Passport and therefore they will need to have this re-issued in their new name. However if you do not currently hold a Passport then you will require a new one to be issued in your new name.
This guide explains what do in both circumstances and a few general notes at the end.
1. I do not already have a Passport but I want one in my new name:
You will need to complete a Passport order form, this can be done online via the Identity & Passport Service (IPS) website - www.ips.gov.uk, by requesting a form through the post (either online or by phoning the IPS on 0300 222 0000) or at an branch of the Post Office which offers the 'Passport Check & Send service'.
You will be required to send certain supporting documents (including two identical 'Passport Photos'), generally this will mean your birth or adoption certificate and your Deed Poll (the original and not a photocopy). We also advise sending a covering letter, downloadable templates are provided for free to our clients.
If you live outside the UK they may require additional documents, please telephone the Passport Adviceline 0300 222 0000 to check or ask at your local Post Office.
Please note that if you complete the form online the IPS will print and send you a copy to sign and return with the required supporting documentation.

Please note that in the case of an order to have a Passport re-issued for a child the IPS will require a letter of consent, signed by the person(s) named on the child's Deed Poll to confirm that consent has been sought from both parents or if not, the reason why. We provide a free downloadable consent letter to our clients which covers this requirement.
3. General Notes
